The Eisner Pediatric & Family Medical Center is the oldest non-profit community clinic in Los Angeles. The Clinic is dedicated to providing high quality medical and dental services, children's day care, and early intervention programs that are affordable to all families regardless of income level. The Center is committed to addressing not only the physical health, but also the social, emotional, familial and developmental issues, as well as the environmental factors influencing development.

The Center's comprehensive scope of services flows from its identity as a "full service" medical home with core objectives including: improving access to quality medical care for underserved populations and to build a family-focused system of care that links social and human services with primary healthcare.

The Atlas Family Foundation Grant provides funding for the creation, development and implementation of an interdisciplinary model for the Strong Families/Healthy Children Initiative with primary focus on the Zero to Three Service Integration Project. The primary targets of ZTSI are infants and toddlers ages 0 – 3 years, along with their parents and caregivers. Secondly, the project focuses on improving the knowledge, skills and attitudes of Eisner physicians, educators, administrators, and support staff. Such improvement is the cornerstone of high-performing integrated systems, as the effective coordination and use of institutional level inputs (resources, facilities, personnel, etc.) in clinical and educational strategies are critical to achieving improved health outcomes.
